John Ivison: Trump has abandoned America's leadership role during the COVID-19 crisis
The announcement by the Liberal government that it will contribute $159 million to fight COVID-19 in poorer countries was couched in its usual hipster social justice blarney – the aid will be in line with its “feminist international assistance approach,” it said.
Sending money overseas will be unpopular in some quarters for more nativist reasons. When the first tranche of assistance was announced last month, Conservative leadership candidate Erin O’Toole tweeted “foreign aid can wait.”
But opponents of an international response are wrong. This is a global pandemic and to subdue it will take global action.
Unfortunately, that is not the conclusion that has been reached by the president of the world’s only indispensable country in a global crisis.
Donald Trump paid lip service to a united front in a G20 leaders’ communique late last month. But every action – from blocking the export of face masks to Canada to trying to buy exclusive access to a German-made vaccine – have belied his professed multilateralism. An order of 200,000 masks bound for Germany was allegedly “confiscated” in Bangkok by the Americans, on the grounds they were produced by a U.S. company – 3M – in China. A German official called it “an act of modern piracy”.
Trump’s supporters will cheer his America First response. But this inward-looking approach is antithetical to the values that have made the United States successful – in Harry S. Truman’s words, a country “built on courage, on imagination and an unbeatable determination to do the job at hand.”
Truman was president when the Marshall Plan was introduced to kick-start Western European prosperity and stop the spread of communism.
After the 2008/09 financial crisis, both the Bush and Obama administrations recognized the need for American leadership on the world stage.
But Trump has relinquished that role, even as he talks about making America great again.
While Trump has slept, China has been busy – donating face masks, test kits, protective suits and expertise to 100 or so countries. At the height of the crisis in Italy, a delegation of Chinese medical experts and 31 tonnes of supplies arrived to help.
According to the Xinhua news agency, there is no geopolitical component to this largesse. Western politicians “gauge the heart of a gentleman by their own standards…. The paranoid should stop politicizing and distorting Chinese assistance,” the state-run propaganda agency said.
However, just because we in the West are paranoid, it doesn’t mean the Chinese aren’t intent on becoming the biggest geopolitical force in the world.
Trump’s abdication of responsibility means that when this is all over, 100 countries will think better of China and worse of the U.S. – including, perhaps, Canada.
As Trump was directing 3M to stop exporting respirators to Canada, the Trudeau government was chartering a cargo flight to China to pick up millions of masks.

Canadian prescriptions for malaria drug with COVID-19 potential surge, some for doctors' own use

Hydroxychloroquine has long been a standard treatment for lupus and rheumatoid arthritis patients. But many of them have reported having trouble obtaining it.
Some Canadian doctors appear to have been snapping up a malaria drug for their own possible use against COVID-19, part of a surge in prescriptions for the medicine that has health-care regulators across the country concerned.
Professional organizations in several provinces have issued statements in recent days discouraging the practice, noting there’s no solid evidence yet that hydroxychloroquine and a related medicine work against the coronavirus.
“As evidence-based clinicians, we must be diligent in our efforts to not let blind hope drive our decisions,” said a joint statement from the associations representing Ontario pharmacists, doctors and registered nurses.

The Quebec Order of Pharmacists, which regulates that province’s druggists, recently urged its members not to fill prescriptions meant for COVID-19 prevention or treatment, calling them “inappropriate.”
The Alberta College of Pharmacists, another regulator, said it has received reports of physicians prescribing hydroxychloroquine for ” ‘office use’ to themselves, to family members and when there is no accepted indication (i.e., treatment of COVID-19 infection).”
Pharmacists in Ontario also received prescriptions “for office use.” It was unclear whether those physicians wanted a supply to dispense to patients — when many clinics were closed — or for themselves and family, said Allan Malek, chief pharmacy officer with the Ontario Pharmacists Association.
“Our message is that we have to exercise a little bit of control here and rational thinking,” he said Monday. “We’re asking for calm in this, and to let the studies unfold.”
Meanwhile, the increased prescription of the malaria medication as a coronavirus therapy is already creating shortages for patients with a proven need to take it, say their advocates.
Hydroxychloroquine has long been a standard treatment for lupus and rheumatoid arthritis patients. But many of them have reported having trouble obtaining it, said Leanne Mielczarek, executive director of Lupus Canada.
“They are experiencing difficulty,” she said. “In some cases, patients have to go to multiple pharmacies to fill their prescriptions.”
The Canadian Rheumatology Association has heard from patients unable to fill prescriptions at all, or told they could only get a 30-day supply, after COVID-19-related prescribing “increased dramatically,” said Ahmad Zbib, the group’s CEO.
That scarcity could ironically affect coronavirus patients, too, should the large trials now in the works conclude that hydroxychloroquine is, in fact, useful in treating COVID-19.
Malek said his research indicates that three of the four approved producers of the medicine in Canada have no supply and the fourth has “very limited” stocks.
Health Canada is currently trying to find manufacturers in other countries who could add to this country’s reserves, he said.
The controversy is unfolding as U.S. President Donald Trump continues to promote the drug — in combination with the antibiotic azithromycin — as a potential COVID-19 “game changer.” On Sunday, Trump stopped Dr. Anthony Fauci, the administration’s chief infectious-disease expert, from answering a reporter’s question about the issue. Fauci has cautioned that more evidence is needed on hydroxychloroquine’s effectiveness against the coronavirus.
Interest in the malaria drug’s potential was first triggered by a small study from France that found it reduced the viral load in some patients, and a larger trial involving 80 patients that lacked a control group — patients who did not receive the hydroxychloroquine/azithromycin combination.
A small Chinese study found some benefit, and another saw no difference between the patients who received the drugs and those who didn’t.
A number of fast-tracked, randomized controlled trials are now testing the medication on much larger groups of patients in Canada and elsewhere. Many experts argue the medicines should not be routinely used for COVID-19 until those studies issue findings in the coming weeks and months.
Echoing a similar phenomenon in the U.S., regulators and professional associations here say they started hearing about a spike in prescriptions late last month.

Demand grew significantly for hydroxychloroquine “in odd doses and quantities, with an off-label and unsubstantiated indication of ‘COVID-19 prophylaxis or treatment,’ ” said the Ontario associations in their joint statement. The result, they said, is a “very serious shortage.”
The regulators for Manitoba nurses, doctors and pharmacists reported a “significant increase” in prescriptions and resulting shortages because of “yet-to-be-proven claims.”
Similar warnings were issued in Alberta.
“We have to do this responsibly and the only was to do this responsibly is to get good evidence, not to speculate,” Greg Eberhart, registrar of the Alberta College of Pharmacy, said in an interview Monday.
B.C.’s regulators predicted that prescribers will face even more pressure from “patients, fellow health care workers and even friends and family” to help them access the drugs.
“Health professionals all have a responsibility … to focus only on evidence-based care and not yield to patient pressure around unproven and potentially dangerous uses of existing medications,” the groups said.

An international poll of thousands of doctors rated the Trump-touted anti-malaria drug hydroxychloroquine the best treatment for the novel coronavirus.
Of the 6,227 physicians surveyed in 30 countries, 37 percent rated hydroxychloroquine the “most effective therapy” for combating the potentially deadly illness, according to the results released Thursday.
The survey, conducted by the global health care polling company Sermo, also found that 23 percent of medical professionals had prescribed the drug in the US — far less than other countries.
“Outside the US, hydroxychloroquine was equally used for diagnosed patients with mild to severe symptoms whereas in the US it was most commonly used for high risk diagnosed patients,” the survey found.
The medicine was most widely used in Spain, where 72 percent of physicians said they had prescribed it.
A debate about hydroxychloroquine was sparked two weeks ago after President Trump touted the drug as a possible “game-changer” in the fight against the COVID-19 pandemic, prompting critics to accuse him of peddling unproven, untested remedies.
To date, “there is no evidence” that any medicine “can prevent or cure the disease,” according to the World Health Organization.
But Sermo CEO Peter Kirk called the polling results a “treasure trove of global insights for policymakers.”
“Physicians should have more of a voice in how we deal with this pandemic and be able to quickly share information with one another and the world,” he said in a press release.
The 30 countries where doctors were surveyed included Europe, South America and Australia — and no incentives were provided to participate, the company said.

Veterans Affairs purchased $168,000 in hydroxychloroquine sulfate tablets from the Colorado-based generic drug distributor Golden State Medical Supply, and an additional $40,000 from the pharmaceutical company McKesson, according to the report.
Procurement records list both as “emergency” purchase orders to combat the coronavirus crisis.
The Bureau of Prisons bought $60,000 in hydroxychloroquine sulfate tablets from the company Premium Rx National on March 31 — and did not mention the deadly virus in its order, but the purchase comes as federal correctional facilities saw a surge in COVID-19 cases, according to the report.
It appears to be the first time the agency has purchased the drug, the outlet reported.
Neither agency responded to questions from the outlet about whether or how their purchases would be used, either immediately or in the future, to treat coronavirus patients.
